The declaration shall include, among others, the following: • General identification and domicile of the creditor.• Address for service of process within the jurisdiction of the Court.• Amount of the claim, including outstanding balances, unmatured amounts, and maturity dates. Principal, interest, and penalties must be itemized separately. In accordance with the suspensive effect provided in Article 54 of Law No. 141-15, the filing of the restructuring petition produces the suspension of certain proceedings, including the accrual of interest, subject to the exceptions established by law. Therefore, a detailed annex must be submitted reflecting the calculation of interest and default charges accrued up to the date of the restructuring petition, broken down on a monthly basis, for verification purposes.• Loan agreements, guarantees, terms, and conditions of the credit.• Priority ranking or privilege whose recognition is requested.• Information regarding any administrative, labor, judicial, or arbitral proceedings related to the claim. The declaration must be duly signed by the creditor and accompanied by copies of supporting documentation (tax invoices, purchase orders, contracts, promissory notes, among others). The creditor must present original documents when required by the Conciliator. If originals are unavailable, the creditor must indicate their location and evidence that the necessary steps have been initiated to obtain them. Undeclared Claims Claims not declared within the established term may be submitted through a late filing, in accordance with the procedure provided in Law No. 141-15. Costs associated with such filing shall be borne by the applicant, unless force majeure or duly substantiated unforeseen circumstances are demonstrated.
